Registration

By enrolling the above named and described dog(s), I represent that I am the legal owner of named dog(s) and I assume all risks, dangers and responsibility caused in whole or part by any other dog, employee of Canine To Five or visitor on the premises. In addition, I understand that I am solely responsible for any harm or damage caused by my dog(s) while he/she is attending Canine To Five.

I understand and agree that Canine To Five and their staff will not be liable for any problems that develop provided reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever.

All dogs must pass a behavior assessment in order to attend Canine To Five Daycare. They must be show no signs of any type of aggression. I understand and agree that in admitting my dog(s),Canine To Five has relied on my representation that my dog(s) is/are in good health and have not harmed or shown aggression or threatening behavior towards any person or any other dog for admittance to daycare or cage-free boarding. Dogs who are boarding in traditional boarding only must not have shown aggression toward any person.

All dogs must be healthy. By enrolling my dog(s) at Canine To Five, I certify that my dog has been free from any condition that could potentially jeopardize other animals or people. Dogs that have been ill with a communicable condition in the last 30 days require veterinarian certification of health to be admitted or readmitted.

I understand and agree that any health problems that develop with my dog(s) will be treated as deemed best by the staff of Canine To Five, in their sole discretion, and that I assume full financial responsibility for all expenses involved.

I have provided a copy of vaccination certificate and proof of last heartworm prevention and flea prevention purchases. All dogs must be current on DHLPP, Rabies and Bordetella. Certificate must have veterinarian\'s name, address and phone number printed on it.

All dogs must have a complete, up-to-date, and approved enrollment form on file. I understand that if my dog has been out of day care for two months or more, re-assessment may be required. All enrollment forms must be completed and signed by the owner prior to admittance.

I understand that if, at any time during care, a dog is observed to have fleas or ticks, treatment will be applied and charged to the owner at the minimum rate of $20.00.

All daycare and cage-free boarding dogs must be spayed or neutered (unless they are under 9 months of age). Any dog demonstrating aggression or other behavior deemed by the staff at Canine To Five may be required to leave the property.

All food brought to Canine To Five must be stored in an airtight container/bag labeled with your dog’s name. Canine To Five is not responsible for loss or damage to any personal items such as leashes, bedding or toys.

I understand that in day care and cage-free boarding, my pet will be spending time with other animals, and an occasional nip or scratch may occur during supervised play.

Pre-paid packages are nonrefundable and expire in 6 months. Fees are due and payable at the end of each day. We accept all major credit cards and cash. Discounted packages are provided only if they are paid in advance. Pre-paid packages are nonrefundable and expire after 6 months of purchase. Our program is flexible enough to accommodate reasonable special needs requests.

Canine To Five reserves the right to refuse admittance to any dog that does not meet the health, temperament or other requirements at the sole discretion of Canine To Five and its employees.

I acknowledge that I have read this Waiver and Release and Liability and indicate my agreement to be bound by its terms and conditions.
